TERMS & CONDITIONS

1. Appointment / Class Attendance;

1.1 – If you are unable to attend an appointment or class, please phone to cancel or re-book with sufficient time to allow this appointment slot to be utilised by another patient.

1.2 – Late cancellation (with less than 12 hours notice) or non-attendance will incur a fee equal to that of 50% of the full service fee, which is the patient’s responsibility and is not claimable through private health insurance or any other third party.

2. Accounts – General;

2.1 – All accounts are to be finalised on the day of treatment.

2.2 – No Credit will be given.

2.3 – Overdue accounts attract administrative fees. The patient will be responsible for all costs incurred in redeeming outstanding monies, including any debt collection, solicitors or court cost will be the responsibility of the patient.

3. Concession Rate;

3.1 – Concession rates are available. Please speak to staff if you believe that you are eligible;

3.2 – Proof of concession is required in order to redeem concession rates.

3.3 – Concession rates are available during 9:00am – 5:00pm – Monday to Friday. Full fee payments are applied outside these times.

4. Private Health Insurance;

4.1 – Private extras health insurers may provide a rebate for Physiotherapy, Remedial Massage and other approved allied health treatments. Please check with your insurer as to your level of cover.

4.2 – The insurer may not cover the full amount of the consult. Should there be an amount owing after the insurers rebate, then this is the responsibility of the patient to finalise on the day of the treatment.

5. Medicare;

5.1 – In most cases, Medicare does not contribute to the cost of physiotherapy services.

5.2 – The EPC (Enhanced Primary Care) Program enables patients with chronic conditions to claim physiotherapy and other allied health services under Medicare, however this must be initiated by your GP. For more information on this program please consult your General Practitioner.

5.3 – Select Health Group Pty Ltd does not bulk bill sessions covered by medicare. Payment in full is required at the treatment session and the medicare rebate can be processed for you through our terminal.

 6. Motor Vehicle Accidents (MVA);

6.1 – If your injuries/condition for which you are seeking treatment were sustained as a result of a motor vehicle accident please speak to our staff, as this practice has non-negotiable responsibilities to the Motor Accident Commission, in relation to fee structure and release of medical information.

6.2 – Select Health Group Pty Ltd will only bill MVA claims to the Motor Accident Commission or the representing insurer upon the claim being approved and treatment confirmed by the insurer to Select Health Group Pty Ltd in writing.

6.3 – Treatment will be administered and billed to the patient at the current Motor Accident Commissions rates upon proof of a pending claim (claim number required). These payments will be required up front at time of treatment by the patient and is then the responsibility of the claimant to seek rebate from the insurer, until such time that Select Health Group Pty Ltd receives written approval ant authority to treat by the insurer.

6.4 – By providing your claim number and details to Select Health Group Pty Ltd, you consent to Select Health Group Pty Ltd, administration or other to search for claim related information and history, in relation to your claim.

7. Return to Work (formerly known as WorkCover);

7.1 – If your injuries/condition for which you are seeking treatment were sustained at work and you are seeking to lodge a claim please speak to our staff, as this practice has non-negotiable responsibilities to Return to Work SA in relation to fee structure and release of medical information.

7.2 – Select Health Group Pty Ltd will only bill Return to Work claims to the Return to Work SA or the representing insurer upon the claim being approved and treatment confirmed by the insurer to Select Health Group Pty Ltd in writing.

7.3 – Treatment will be administered and billed to the patient at the current Return to Work SA rates upon proof of a pending claim (claim number required). These payments will be required up front at time of treatment by the patient and is then the responsibility of the claimant to seek rebate from the insurer, until such time that Select Health Group Pty Ltd receives written approval ant authority to treat by the insurer.

7.4 – By providing your claim number and details to Select Health Group Pty Ltd, you consent to Select Health Group Pty Ltd, administration or other to search for claim related information and history, in relation to your claim.

8. Patient Confidentiality;

8.1 – Select Health Group Pty Ltd, has taken every precaution to ensure that information passed on to administration or treating therapist by the patient is held under strict confidentiality.

8.2 – The information that is held will not be passed onto a third party without the prior consent of the patient or their legal gaurdian.

9. Marketing;

9.1 – By issuing your contact details to Select Health Group Pty Ltd, you agree to receiving information relating to treatment, special offers, appointment reminders or any other marketing that Select Health Group Pty Ltd, may distribute from time to time.

9.2 – It is not our intention to send out inappropriate information or amounts of marketing materials, however should you believe information sent is inappropriate or wish to no-longer receive these materials then it is requested that you contact us on 82404577 or email admin@selecthealthgroup.com.au and we will ensure where possible to remove you from our marketing list as soon as possible.
